Description: We provide primary health care, social services and substance abuse treatment to eligible homeless men, women and children. We also provide limited primary care to eligible Latino people. We provide immunizations, physicals, preventive and acute medical care. Our substance abuse services are a 12-step based intensive outpatient model for adults. Our social services include assisting people with finding temporary and emergency shelter, transitional housing and limited assistance with permanent housing. We also assist people with obtaining various forms of identification and with applications for various types of entitlement programs. We have staff, both medical and social services, who provide outreach and go to sites where homeless people may be found. Actual street address: 717 East 11th Street, Chattanooga, TN 37403
